Application forms let you decide what information you collect from candidates, and keep it consistent across every job.

An application form is a layer on top of your Custom fields. A custom field defines what a field is — its name and its type. The application form decides where that field appears, whether it is required, and in what order candidates see it.

You set company-wide defaults once in settings, and every job inherits them. Recruiters can then adjust the fields for an individual job.

Go to Settings → Application forms to configure.

The four form templates

The Application Form Templates page lists four templates. Each one controls a different way candidates enter your ATS, so you can ask agencies for different details than you ask candidates applying on your careers page.

Candidate Application Form Candidates applying through your careers page
Employee Referral Form Employees referring a candidate
Agency Submission Form Agency partners submitting candidates
Team/Internal Candidate Form Recruiters adding internal or sourced candidates manually

The Fields column shows how many fields are currently active on each template. Click any row, or the pencil icon, to open it.

Set field visibility

Each field on a template has three states. Click a state to apply it.

State What it means
Off The field is not shown on this form at all
Optional The field is shown, but the person can skip it
Mandatory The field is shown and must be filled before the form can be submitted

There is no separate "hidden" state — setting a field to Off hides it.

Reorder fields

Drag a field by its handle to change the order. Candidates see the fields in exactly the order you set here.
